He was predeceased by his parents, J.E. & Elizabeth Cook and brother, Tony Cook.

Survivors include his brother, Tommy (Claudia) Cook.

A graveside service will be held on Saturday, January 2, 2016 in Tyner Cemetery at 2pm.
