Jerry Lynne Givens, 60, of Ooltewah, TN was born on May 16, 1956 and died December 19, 2016.

Survivors include his sons, Michael Claramunt and Steven Givens and Daniel Givens; daughter, Ashley Phillips; granddaughters, Phoenix and Alannah; brother, Michael Givens; sister, Rhonda Davis; parents, Anna & J.C. Givens with significant other, Patricia Moore.

The family will receive friends on Tuesday, December 27 at Spring Creek Church of God from 11am 2pm. The Rev. Bennie Mills will conduct the service at 2pm in the church. Interment will follow at Lakewood Memory Gardens, South.
